The programme aims to encourage practitioners to further develop their knowledge and skills in order to maximise person centred care during routine practice and develop services to support individuals with long term conditions’ in line with health and social care policy. It will provide students with an opportunity to debate and evaluate evidence based, person centred strategies and/or interventions (such as falls management) for active ageing, self-management and independence, in an inter-professional context. Students will critically evaluate the application of these approaches in their own individual practice area.
